CSLA Program

In order to fulfill PHI's goal of transitioning people into community-based living, this program has been established to give clients independent living with access to dependable and knowledgeable support staff. PHI's focus is on teaching the skills needed to participate in everyday events of life. Our CSLA goal is to enable clients to ultimately care for themselves and therefore gain confidence in there abilities.

The main cornerstones of the CSLA Program are: The person/guardian, through a person-centered planning process, identifies the supports and services which best meet his or her needs. If provider agency staff and the person receiving the services have a parting of ways, it is the agency that must be replaced, not the person.

The person must live in his or her own home (including an apartment, condominium or house, owned or rented) where the setting is controlled by the person/guardian and not a service provider. Control may be shared with other people who live in the home. No more than three unrelated people excluding live-in care givers may reside in the home for it to qualify as an allowed living arrangement for CSLA. Participants may also reside with their families or with a relative.
